Policies and Guidelines
Scheduling Appointments

When you schedule an appointment with me, I set aside that time specifically for you. I do my best to allow time between sessions to be ready for you when you arrive.

I can schedule your appointment over the phone, by text, or by email. For new clients, I will contact you via phone before your first visit. Credit cards are required in order to reserve your appointment time. Please view the cancellation policy, below. You will be asked to sign a credit card authorization form at your first visit.

For regular clients, appointments over the phone are scheduled immediately. It may take more time to schedule an appointment if text or email is used. When you receive a reply that includes a date and time for an appointment request through text or email, I will hold that day/time for 12 hours. If I do not receive a confirmation from you within 12 hours, that time will be forfeited.

If you are a new client, you will be sent two emails from Restorative Hands Massage & Wellness prior to your first appointment: an appointment confirmation and a second email asking you to complete your client intake form. Please fill out your health history form at least 24 hours before your appointment to give me time to review, prepare, and so that you can receive your full session time as scheduled.

All clients: please arrive 10 minutes prior to your session time to ensure you get the full session time and to allow for a relaxed and unhurried experience.

Your session is scheduled to begin at the start of your appointment and your time on the table is the full session time. I allocate enough time between sessions for you to get on and off the table and for us to have a pre/post conversation.

Massage appointments may be scheduled for office or home sessions. There is an additional travel fee of $25 for home appointments.
Cancellations/Rescheduling

Restorative Hands Massage & Wellness asks that you give a 24-hour notice of cancellation or rescheduling if you cannot make your appointment. This allows me the opportunity to fill your scheduled appointment. Cancelling or rescheduling of an appointment in less than 24 hours will result in a charge of the cost of your full scheduled session time to your card on file. I will notify you whenever I need to charge your card on file.

Circumstances that are beyond your control (family emergencies, cancelled daycare, sudden illness or injury, etc.) will be handled on a case-by-case basis.

If you fail to cancel within 24 hours multiple times (2 or more), you may be asked to pre-pay for future sessions.

Please do not rely on a courtesy reminder text, email, or phone call. It is still your responsibility to know when you are scheduled for a session.
Late Arrivals

I understand that unanticipated issues occur. Recognizing that I do reserve the scheduled time just for you, I have other clients to consider and must maintain smooth session transitions.

I regret that late arrivals may not receive extension of a scheduled session. If you arrive on time or late, I can only give you whatever time remains of your appointment. In only special cases, and when my schedule will allow, I may be able to accommodate a full session. This will be at my discretion and only with proper, advanced notification. The original full session time will be charged.

In return, I make every effort to run on time for you. If I need to cancel your appointment within 24 hours of its scheduled time due to illness or an emergency, I will make every effort to notify you as soon as possible.
No-Shows

If you either forget or consciously choose to forgo your appointment for whatever reason, you will be considered a “no-show.” You will be charged for the cost of your full scheduled session time without exception.

If you're a “no-show” more than once, you will need to pre-pay for future sessions.

Please just call; I worry about my clients' well-being if I don't hear from you!
Illness

If you are ill or do not feel well either within 4-5 days of your appointment or the day of your appointment, please contact me so we can discuss whether you should be receiving a massage. Some illnesses are contraindicated for massage.
Unacceptable Behavior

Restorative Hands Massage & Wellness is a professionally licensed establishment. Requests of any sexual or degrading nature will result in early termination of the session. This includes: requests for less draping, suggestive comments, and unmerited physical contact. You are still responsible for the full amount of the session.
Payment

Restorative Hands Massage & Wellness accepts cash, checks, and credit cards for payment of services. If a check or credit payment is returned unpaid from your bank or other facility, you will be charged a $20 return fee and you will be responsible for re-payment in cash.
Communication

In order for you to receive the best massage therapy possible, please communicate ANYTHING and everything, including your needs, preferences, requests or feedback, at any time before, during, or after your massage. Immediate feedback is crucial during your massage. If you feel distracted, unwell, too hot/cold, or uncomfortable at any time during the session, please let me know so that I can make adjustments. I want your HONEST feedback, positive or negative, and promise to not be offended.
Privacy Policy
Privacy of personal information is an important principle to me, Ingrid Kania, LMT, Owner of Restorative Hands Massage & Wellness. I’m committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the goods and services I provide. This document describes my privacy policies.
WHAT IS PERSONAL INFORMATION?
Personal information is information that identifies individuals. Personal information includes information that relates to:

an individual's personal characteristics (e.g., gender, age, home address or telephone number, family status);

WHO I AM
Restorative Hands Massage & Wellness is the business name for me, Ingrid Kania. I’m a licensed massage therapist in CT (#9377) and TX (MT130678). I provide massage therapy and reiki services to the general public.
PRIMARY PURPOSES FOR COLLECTING PERSONAL INFORMATION:
Like all medical professions, I collect, use and disclose personal information in order to serve my clients. For my clients, the primary purpose for collecting personal information is to provide treatment. For example, I collect information about a client's health history, including their family history, physical condition, function and social situation in order to help me assess what their health needs are, to advise them of their options and then to provide the health care they choose to have. A second primary purpose is to obtain a baseline of health and social information so that in providing on-going health services I can identify changes that occur over time.
I use Massagebook to store and maintain client records. Massagebook is HIPAA compliant and data is stored in secured servers.
RELATED AND SECONDARY PURPOSES FOR COLLECTING PERSONAL INFORMATION:
Like most organizations, I also collect, use and disclose information for purposes related to, or secondary, to my primary purposes. The most common examples of my related and secondary purposes are as follows:

To invoice clients for goods or services that was not paid for at the time, to process credit card payments or to collect unpaid accounts.

Clients have the right to access and review their records and client information is retained for the HIPAA standard of 7 years following last contact.
BROWSING MY WEBSITE:

I do not collect personally identifiable information when you browse my website. However, information such as time of day, browser type and IP address may be collected in order to provide a better website experience
PROTECTING PERSONAL INFORMATION:
I understand the importance of protecting personal information. For that reason, I take the following steps:

Paper information is either under supervision or secured in a locked or restricted area.

Electronic hardware is either under supervision or secure in a locked or restricted area at all times. In addition, passwords are used on computers.

Electronic exchange of information is avoided. If necessary, information is encrypted and identifiers are removed.

I collect, use and disclose personal information only as necessary to fulfill my duties and in accordance with this privacy policy.

Clients are required to sign an authorization form to give permission for me to communicate with their other health care providers. This authorization may be revoked at any time by contacting me at ingridkanialmt@gmail.com.

Clients may elect to receive email or text appointment reminders. You may unsubscribe from these reminders at any time by emailing me at ingridkanialmt@gmail.com or verbally requesting your removal.

RETENTION AND DESTRUCTION OF PERSONAL INFORMATION:

In accordance with HIPAA security and privacy policies, I retain client record for 7 years after the last contact.

Electronic information is destroyed by deleting files and paper information is destroyed by shredding.
YOU CAN LOOK AT YOUR INFORMATION:
You have the right to access your records that contain information about your treatment. You have the right to request a correction to your information as applicable to factual information and not my professional assessment.
